1. Please download and complete the following documents:

• Application Process Booklet
• Pre-Application
• Authorization and Release of Information

2. Return the Pre-Application and Authorization form to the Habitat office at 540 Lycoming Street, Williamsport PA.  Please note:  the Pre-Application must be accompanied by $5.00 credit check fee for each adult who is applying.
